If you do not know the exact time an incident occured, please use your best judgement to ascertain an approximate time.

A Red incident is an emergency situation that has compromised volunteer safety or van integrity. This requires IMMEDIATE action from your Service Leader and OSA HQ.

An Amber incident requires action from your Service Leader prior to the next shift.

A Green incident is a 'good to know' with no urgent follow-up action required.

Please describe any actions you, or anyone else involved in the incident took. E.g. packed up service, called emergency service, notified service leader, congregated at safe place or applied first aid.
E.g. "Volunteer X" was in tears during the incident and seems quite traumatised, "Volunteer X" got a small cut on his hand which did/did not require attention.
E.g. suspend the service, develop new procedures, provide additional training for volunteers, change the conditions under which we operate (lighting, timing, location)
